Carolina Companion Care LLC is seeking a compassionate, skilled, and dedicated Nurse to join our team on an as-needed basis to conduct assessments for our valued clients. Position Overview :

The Nurse will be responsible for conducting thorough assessments of our clients to develop personalized care plans, ensuring that all health needs are met with professionalism and empathy. This is a flexible, as-needed position, ideal for someone looking to make a meaningful impact while enjoying a work-life balance.

Key Responsibilities :

Conduct comprehensive health assessments of clients in their homes.

Document findings and create care plans tailored to each individual’s needs.

Collaborate with caregivers and other healthcare professionals to ensure optimal care.

Monitor and update client health records.

Provide guidance and support to clients and their families regarding care protocols.

Qualifications :

Current, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in [state].

Minimum of [X] years of nursing experience, preferably in home care or healthcare assessments.

Strong assessment and critical thinking skills.
