A company is looking for an Analyst, Health Coach (Remote).
Key Responsibilities
Educate members on health topics and empower participation in group coaching
Assist members in engaging with healthcare providers and care management teams
Document member interactions and follow up on personal action plans
Required Qualifications, Training, and Education
At least 2 years of experience as a Health Coach
Experience in care management, healthcare, social work, or aging network
Associate's Degree required; Bachelor's preferred
Health Coach Certification preferred
Bilingual candidates are preferred
