In this pivotal role, you will directly supervise and train primary care providers (PCPs) within your center, holding responsibility for enhancing clinical quality, efficiency, patient outcomes, and satisfaction. While you will maintain a portion of your time for direct patient care, your leadership duties will focus on fostering a vibrant clinical-operations synergy.
Key Responsibilities:
- Provide comprehensive care for patients with both acute and chronic illnesses, primarily in older adults.
- Take full accountability for patient care and outcomes while coordinating with specialists when necessary.
- Actively engage with the hospitalist for patients during hospital stays, ensuring continuity of care.
- Lead a dedicated care team, which includes medical assistants, care facilitators, and care coordinators.
- Coordinate care for patients unable to visit the office, collaborating with transitional care teams and other healthcare resources.
- Facilitate weekly Super Huddles and Transforming Care Meetings, supporting the clinical leadership as needed.
- Contribute to center management, recruiting for both patients and additional clinical providers.
- Mentor and coach PCPs to achieve outstanding clinical outcomes, ensuring best practices through regular huddle participation.
- Collaborate with operations directors to enhance financial performance and increase patient membership.
- Perform additional duties as assigned to meet the center's operational needs.
Qualifications:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty is required.
- Active MD licensure in your State of employment is necessary.
- At least 1 year of clinical experience in geriatrics, adult, or family practice is preferred.
- Completion of ChenMed training and board certification in your specialty is preferred.
- Current DEA number is required for prescribed controlled substances.
- Basic Life Support (BLS) certification must be obtained within the first 90 days of employment.
Compensation:
Salary range is $238,832 - $341,189.
