OTR | Licensed Occupational Therapist

We are seeking a compassionate and skilled Licensed Occupational Therapist to join our team in the Midwest region. In this role, you will evaluate, treat, and monitor patients while providing ongoing assessments to support their rehabilitation journey. You will be responsible for developing individualized treatment plans, maintaining clear communication with patients, families, caregivers, and healthcare providers, and supervising therapy assistants and support staff.

Essential Job Functions

• Conduct thorough patient evaluations and develop treatment plans using approved procedures.
• Interpret findings and communicate plans effectively to patients, families, and care teams.
• Prepare timely documentation including evaluations, progress notes, discharge summaries, and logs in compliance with federal and state regulations.
• Collaborate with rehabilitation team members to coordinate comprehensive care services.
• Create and implement home programs to enhance patient performance in their environments.
• Develop evidence-based treatment protocols and manage referrals from licensed healthcare professionals.
• Maintain positive relationships with patients, families, caregivers, and staff, demonstrating a service-oriented approach.
• Monitor patient progress and adjust treatment plans as needed to ensure quality care.
• Train and supervise occupational therapy assistants, assist with orientation of new staff and students.
• Manage equipment maintenance and supply inventory efficiently.
• Support quality assurance initiatives and complete required facility reports.
• Adhere to all safety, confidentiality, and compliance policies, including HIPAA regulations.
