About Facility:
o Home Health
o Radius rule: Traveler's permanent address must be 50 miles outside of home unit
Department Information:
o Will make visits throughout University City, Cornelius, Davidson, Kannapolis, Salisbury, Concord, Huntersville, and cities in between
There is not a max mileage to drive. The average is 60-80 miles a day. Mileage is reimbursed weekly.
Best central location for housing University City and/or Concord but must be comfortable to float throughout the areas above.
o Teammates are required to complete 6 visits a day (routine visits, discharge, ROC, recert, reassessment)
o Minimum Years of Experience required? 1 year Home Health Nursing
o Will you accept a first-time traveler? Yes, if seasoned to home health
Patient Types/Common Diagnoses:
o Chronic diseases, wound care/wound vac, IV, pleurx
Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:
o Driver's License and proof of Insurance
o BLS
Skills required:
o IV, pleurx, wound vac, wound care, assessment, disease teaching, OASIS, case management (ability to oversee/episode management up to 25-30 patients at once)
Support within the Department:
o Departmental leadership
o Schedular
o Clinical Manager
o Clinical Supervisor
o Peers/Clinical Team
Technology/Equipment:
o EPIC Rover - view only
o Focura - will upload discharge instructions and other important docs such as wound pictures
o Samsung note 9 phones
o Home Care Home Base
Orientation (facility and unit):
o NEO
o 5 days orientation with Preceptor
Scheduling:
o Weekend rotation: none, unless traveler is willing and interested in working weekends
Unless posted for Weekend only Work
o Call is not required, but may be available if interested
o Holiday Expectations: Travelers will be expected to work all holidays, unless approval is received from Manager
o RTO: Manager approval
o Schedule cycle: every 13 weeks
