Job Summary :

Under the supervision of the Team / Program Manager, the HH clinician is accountable for effectively managing or caring for a caseload of patients from admission to transition into the community, involving interdisciplinary support and referrals when appropriate. This position coordinates the interdisciplinary care, monitors and adjusts treatment to achieve positive therapeutic outcomes, and identifies opportunities for process improvement. Requirements :

Equivalent education required for RI licensure required.

Bachelor’s Degree in relevant discipline strongly preferred.

A minimum of one (1) year progressive clinical experience required. Previous experience in an acute care or Home health care environment setting preferred.

Valid CPR certification required.

Valid RI Board license.

Valid driver’s license and automobile liability insurance are required.
