Your Responsibilities :

• Evaluate and assess patients' physical abilities to develop comprehensive, individualized treatment plans.
• Implement and monitor physical therapy interventions to ensure optimal patient outcomes.
• Collaborate with a multidisciplinary team to facilitate coordinated patient care.
• Document all patient interactions and treatment in the electronic medical record (EMR) system.
• Educate patients and their families about rehabilitation plans and home exercise programs.
