Using a Housing First approach, Avalon's Support Coordinators provide outreach, intensive case management and housing support services to Avalon tenants and clients. The individuals we serve face many challenges, including mental illness, substance use disorders, and/or chronic medical conditions in addition to chronic homelessness. Our clients live in Avalon Housing units, with private landlords or in public housing located within Washtenaw County.
Support Coordinators collaborate with community partners to coordinate delivery of a broad range of services that meet the mental, physical, psycho-social and housing needs of our clients, either through direct service or connections to appropriate resources.
