Social Services Director

Position Overview:
As a Social Services Director you will help oversee care planning and ensure residents’ psychosocial needs are met with compassion and professionalism. You’ll act as a vital bridge between residents, families, facility staff, and community resources.

Responsibilities:

• Assess and address the social and emotional needs of residents

• Develop and manage individualized care and discharge plans

• Serve as a liaison among residents, families, medical staff, and external agencies

• Maintain accurate documentation, including MDS 3.0 and CAA’s

• Advocate for residents' rights and support ethical care practices

• Coordinate psychological services and hospice referrals as needed

• Participate in interdisciplinary care planning and quality improvement efforts

• Ensure compliance with state and federal regulations regarding social services

What We’re Looking For:

• Licensed Social Worker (LSW) – Preffered

• Experience in a skilled nursing or long-term care setting preferred

• Strong communication, organizational, and leadership skills

• Knowledge of community resources and elder care best practices

• A passion for working with seniors and making a meaningful impact
