Schedule : Monday – Friday, 8 : 00 a.m. – 5 : 00 p.m.

Salary Range : $66,000 – $80,000

Position Summary

We’re looking for an Associate Marriage and Family Therapist (AMFT) to support our Integrated Behavioral Health Program in San Leandro, CA. This role provides an opportunity to gain real-world experience in an interdisciplinary setting that bridges mental health and primary care. You’ll receive direct clinical supervision while helping families access the care they deserve.

Key Responsibilities

Care Coordination & Program Support

• Deliver in-person and telehealth-based care coordination for individuals with behavioral health and social support needs
• Conduct comprehensive client assessments and contribute to individualized care planning
• Facilitate warm handoffs and referrals to behavioral health, medical, housing, and community-based resources
• Monitor engagement and support follow-up care to maintain service continuity
• Collaborate with internal care teams and external partners to address client needs holistically
• Participate in multidisciplinary team meetings, case reviews, and service planning sessions
• Document services and progress accurately in the electronic health record (EHR)
• Assist with data collection and service tracking to meet reporting requirements
