Job Title : Field / Home Health RN

Location : Brooklyn, Bronx, Orange County, Westchester, Nassau County - New York NY

Job Type : Full-Time (Permanent)

Shifts : 8-10hours, Can schedule their own timings by calling patients.

Requirements :

Active RN License
Valid Driver's License and must have their own vehicle.
Previous experience preferred, New Grads are welcome and will be provided training.

Responsibilities :

Provide in-home nursing care, including assessments, treatment plans, and medication administration.

Monitor patient health status and progress, adjusting care plans as necessary.

Educate patients and families on managing illness and post-treatment care.

Collaborate with healthcare team to ensure continuity of care.

Maintain accurate and timely clinical documentation.

Travel to patient homes within assigned territory.
