Flushing- Queens, NY- Mon-Fri – 8 : 00am – 3 : 00pm
Must be Bilingual- Spanish.
Hourly Rate - $43.00

Summary :
As a vital member of our dedicated Children’s Services team, you will provide compassionate support to children with illnesses and disabilities, helping families access essential services and develop strategies to thrive. Your expertise will foster resilience, promote independence, and create a brighter future for children and their families.
Responsibilities :
- Conduct initial evaluations and ongoing assessments to understand each child’s unique needs.
- Develop personalized social service plans and recommendations to support children’s development and well-being.
- Assist families in obtaining vital resources such as housing, Medicaid, respite care, assistive technology, and recreational programs.
- Collaborate with interdisciplinary teams to plan, implement, and monitor Individualized Family Service Plans (IFSPs) and Individualized Education Programs (IEPs).
- Act as a trusted advocate, facilitating communication between families, schools, and service providers.
- Conduct home visits, attend team meetings, and participate in professional development.
- Maintain detailed documentation of all cases, ensuring confidentiality and compliance.
- Support student transitions and participate in community and in-service training activities.
Qualifications :
Master’s Degree in Social Work from an accredited program.NYS Licensed Master Social Worker (LMSW) certification.Prior experience working with children and families preferred.Excellent interpersonal, organizational, and communication skills.Ability to work independently, handle multiple priorities, and maintain strict confidentiality.Flexibility to work varied hours and travel for home visits.A compassionate, proactive attitude with sound judgment and problem-solving skills.
