[job_card.job_description]Delivering direct patient care in alignment with personalized care plans and physician orders. Monitoring vital signs, symptoms, and patient responses to ensure optimal care. Collaborating closely with Registered Nurses to report findings and implement care plans effectively. Building meaningful connections with patients by understanding their physical, mental, and emotional needs for a successful recovery. Celebrating each patient's progress as they achieve their recovery milestones. Current LPN / LVN licensure as required by state law. CPR certification is essential. Preferred experience includes one year in an inpatient medical-surgical or general hospital setting; rehabilitation experience is a plus.
