In this role, you will play an essential part in ensuring a smooth transition through their acute care stay with a focus on progressing towards discharge.

Your responsibilities will include :

• Delivering and coordinating patient care with the caregiving team.
• Ensuring that all patients receive competent and compassionate care.
• Working collaboratively with referring facilities, hospital staff, and patients in various settings including inpatient, ambulatory, and community.

This position offers a starting hourly rate of $33.00, with a maximum of $33.99, depending on experience.
