[job_card.job_description]Salary : Up to $130,000 annually, based on experience Full IRS Mileage Reimbursement at $0.70 per mile Comprehensive benefits package including 401(k), Paid Time Off, and Medical Insurance with dental and vision coverage Paid New Hire Training (3 to 5 days) 24 / 7 Clinical and Intake Support Must possess a valid Physical Therapist license in the State of Arizona. Current and valid BLS (CPR) and ACLS certifications Negative TB test result and flu shot within the past year Up-to-date immunization / vaccination records including COVID-19, Hepatitis B, MMR, TDaP, and Varicella Valid Drivers License with auto insurance

We are seeking a compassionate and skilledPhysical Therapist (PT)forfull-timeposition to join our growing home health care team. In this role, you will provide in-home therapy services to help patients improve mobility, function, and overall quality of life. You will evaluate patients, develop individualized therapy plans, and accurately document care and progress in accordance with agency policies.
Employment Type : Full Time or Per Diem
Area Coverage : West Valley Phoenix area (Glendale, Peoria, Surprise, Goodyear and surrounding areas)
