LPN Intake Coordinator

We are hiring for an LPN Intake Coordinator.

Full-time position pay range $33.00 - $33.50 / hour depending on experience.

Responsibilities

The LPN Intake Coordinator receives and analyzes incoming referral and order calls while effectively communicating patient / referral information to appropriate teams.

Obtains demographic and clinical information from clients, physician’s offices, and other referral sources to compile referrals.

Verifies new referring physician licenses on referrals taken in department.

Researches coverage or pay source for each referral and verifies commercial policies and HIQA.

Educates prospective patients, families, physicians, and hospitals regarding home health services available.

Serves as a liaison between clinical and marketing staff.

Qualifications

License Requirements

• Current LPN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR Certification required.
• Minimum of one (1) year experience in medical office procedures / intake.
