Serving patients since 1980, UChicago Medicine AdventHealth GlenOaks features 143 licensed beds, a Level II trauma center, and a therapeutic day school and transition program. Our key service lines are orthopedics, pain management, oncology, surgery and our Birth Center. UChicago Medicine AdventHealth Glen Oaks is the only disproportionate-share hospital in DuPage County. Performs basic patient monitoring activities under the supervision of a Registered Nurse (RN). Coordinates referrals from interdisciplinary team members and facilitates patient care management across the continuum. Ensures quality patient care and regulatory compliance. Participates in daily multidisciplinary rounds to review the progression of care and discharge plans for all assigned patients. Remains at the bedside of assigned patients at all times unless relieved by an appropriate staff member. Assists in calming and reorienting patients in cases of agitation or confusion. Removes visible hazards and broken equipment from the immediate environment and reports as appropriate. Records all observations accurately and promptly. Provides support to patients with complex psychosocial needs under the guidance of a Care Management mentor. Performs other duties as assigned.

The expertise and experiences you'll need to succeed : High School Grad or Equiv Required. Work Experience Preferred.
