Licensed Vocational Nurse (LVN) Job Duties:

Perform care as defined by clients individual Plan of Care and within scope of states nurse practice act
Monitor patients condition and notify appropriate personnel of any changes
Providing treatments, and working collaboratively with the members of the team to help meet positive client care outcomes
Provide accurate, detailed, and timely documentation consistent with the plan of care and the nursing process
Conduct ongoing assessments
Participate in health/therapeutic counseling, teaching and emotional support to the client and family/caregivers
Prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous and intramuscular medications. Observe, report, and record adverse reactions to medications and/or treatment to the registered nurse
Perform wound care and recognize the need for and institute emergency measures in a rapid and appropriate manner
Follow principles of infection control and Universal Precautions
Treat clients, family, and staff with courtesy and respect

Requirements:

Current unrestricted LVN license in state of Texas
1+ years of experience on your license required
1+ years' Home Care experience preferred
Reliable transportation / valid drivers license / auto insurance
Current CPR/ BLS
Negative TB test
