This care is based on nursing theory and research for a designated group of patients. The nurse delivers patient care by integrating professional practice and nursing processes. Assessment and evaluation of patient care needs are performed while applying critical thinking skills. Relevant assessment and intervention skills are utilized in the delivery of nursing care. Collaboration with the interdisciplinary healthcare team is essential in developing and implementing the plan of care. It is important to prioritize patient care, teaching, rounding, and planning after hospital care. The nurse educates patients and caregivers about their plan of care, promoting health and preventing disease. Effective communication with patients, families, and the interdisciplinary patient care team is crucial. Assignments are managed within a care delivery model and resource allocation is demonstrated effectively.
