About the Role :

As a Nurse Practitioner, you will conduct essential assessments during visits that last up to one hour, gaining crucial insights into each member’s health. Your responsibilities include :

• Conducting comprehensive adult / geriatric assessments, covering medical history, diagnosis, treatment, and necessary referrals.
• Administering point-of-care and lab testing as required.
• Collaborating with physicians, case managers, social workers, and key caregivers as necessary.
• Working closely with the Primary Care Physician for patient education and follow-up.
• Providing services in members' homes or Skilled Nursing Facilities.
