About the Role :
As a Nurse Practitioner, you will conduct essential assessments during visits that last up to one hour, gaining crucial insights into each member’s health. Your responsibilities include :
- Conducting comprehensive adult / geriatric assessments, covering medical history, diagnosis, treatment, and necessary referrals.
- Administering point-of-care and lab testing as required.
- Collaborating with physicians, case managers, social workers, and key caregivers as necessary.
- Working closely with the Primary Care Physician for patient education and follow-up.
- Providing services in members' homes or Skilled Nursing Facilities.

Master's Degree or equivalent experience with NP licensure.Current NP licensure with prescription authority in your state.AANP, AACN, or ANCC board certifications in relevant specialties.Current BLS, ACLS, or CPR certification.1 year of NP experience preferred, though new grads are encouraged to apply.Valid driver's license and proof of automobile insurance.Strong computer skills and familiarity with electronic health record software.Excellent communication skills, both verbal and written.The Matrix Culture :
