Shift Information : Monday-Friday 10 AM - 7 PM

Your Responsibilities :

Conduct pre-admission evaluations to determine service appropriateness.

Guide and educate patients and families about our services throughout the admission process.

Secure informed consent from patients or their representatives.

Perform nursing assessments and establish initial care plans focused on enhancing comfort and quality of life for patients and families.

Qualifications :

Graduation from an accredited nursing program.

Current registered nurse license in the state of practice.

Active CPR certification.

Preferred experience of at least 1 year in hospice care.

Ability to make independent decisions when necessary.

Effective communication skills to interact tactfully with patients, families, and colleagues.

Knowledgeable about quality assessment and assurance procedures.
