Overview

Join our client's dedicated Home Health team as a Physical Therapist, providing high-quality care to patients in their homes. In this role, you will assess patient needs, develop and implement personalized treatment plans, and work closely with caregivers to promote safety, independence, and improved physical function.

Key Responsibilities

Perform comprehensive physical therapy assessments and create individualized treatment programs

Monitor, document, and report patient progress and response to therapy

Educate patients and caregivers on exercise programs, mobility aids, and safe home practices

Supervise physical therapy assistants and certified nursing assistants as appropriate

Maintain accurate clinical documentation and participate in interdisciplinary meetings

Schedule and Shift Details

Monday Friday, day shift (start and end times vary based on patient needs)

One weekend every 6 weeks (Saturday and Sunday day shift)

Two holidays per year (one summer, one winter rotation)

No on-call requirements

Travel

Local travel to patient residences

Qualifications

Required

Master's degree or higher in Physical Therapy from an accredited institution

Valid Physical Therapist license in Illinois (IL)

Minimum of 1 year of experience in a healthcare setting

Valid driver's license and reliable transportation

Preferred
