Job Title : Registered Nurse (RN)

Shift : Days - 8 hours, Monday to Friday, 9 AM - 5 PM, every 4th weekend on call

Experience : Minimum of 12 months of hospice experience in home settings within the past 3 years. Must have experience in case management and admissions.

License : Valid RN License

Must-Have : Solid experience with Homecare Homebase (HCHB) software

Job Description :

• Manage a caseload of 14-18 patients
• Complete patient admissions efficiently
• Provide compassionate hospice care in home settings
• Collaborate with a multidisciplinary team to ensure optimal patient care
• Work every 4th weekend on call
• Belong to the Blue Color team for territory management (refer to dashboard map for details)
