[job_card.job_description]Conducting thorough initial and ongoing assessments to monitor patient progress. Evaluating changes in patient status during each visit. Adjusting treatment plans as needed to best serve the patient's needs. Documenting all treatments and skilled interventions provided. Educating patients and their families to support achieving therapy goals. Active Physical Therapist licensure in Arizona. Minimum of one year of Physical Therapy experience. Experience in a home care setting is preferred. CPR certification required (demonstration course is needed). Valid Arizona Fingerprint Clearance Card. No Covid vaccination requirement.
Home Health Physical Therapist Opportunity
Status : Part-Time (Minimum expectation of 5 visits or 10 hours per week)
Rates : $65 - $90 per visit based on experience, plus mileage and travel time.
