Position Summary

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN / LPN) to provide high-quality skilled nursing care to patients in their homes. This is an excellent role for nurses who enjoy autonomy, hands-on patient care, and the ability to make a meaningful impact in a home setting.

As a Home Health LVN, you will deliver skilled nursing interventions under the supervision of an RN Case Manager, monitor patient conditions, provide education, and support the interdisciplinary team to ensure optimal outcomes. This role aligns with CMS Home Health Conditions of Participation and typical home-based nursing standards.

Key Responsibilities

• Provide routine skilled nursing visits as assigned by the RN Case Manager.
• Perform simple wound care and dressing changes under RN supervision, according to the established care plan.
• Administer medications and injections as ordered.
• Monitor and document vital signs and changes in patient condition.
• Assist with catheter care, ostomy support, and chronic disease monitoring.
• Reinforce patient and caregiver education provided by the RN.
• Communicate any clinical changes promptly to the supervising RN.
• Complete timely, accurate visit documentation in accordance with agency policy and CMS requirements.
• Follow the established plan of care and collaborate with the RN in updating patient needs.
• Communicate effectively with the interdisciplinary team, including physicians, therapists, social workers, and aides.
• Participate in case conferences and provide relevant updates regarding patient progress.
• Provide care in alignment with the Home Health Conditions of Participation, state practice regulations, and agency policies.
• Maintain infection control practices and follow safety protocols.
• Attend required trainings and maintain competency for all delegated skills.
