Care Navigator

A company is looking for a Care Navigator (Remote).

Key Responsibilities

Conduct telephonic outreach to members and providers for service coordination and documentation

Coordinate services and manage scheduling with providers, community partners, and caregivers

Support member and provider requests, ensuring timely responses and issue resolution

Required Qualifications

High School diploma or GED required

One (1) year of related experience in a health-related service field required

No specific licensure or certification required

Proficiency in Microsoft Office Suite

Ability to work independently and manage time effectively
