Gardner Health Services is currently recruiting to fill the following two (2) Lead Care Manager positions :

• Full-time, 40 hours / week position based out of the Downtown Resources Center located in San Jose, California
• Full-time, 40 hours / week position based out of the Gardner / Packard Clinic located in Atherton, California

As a member of the Gardner Downtown Resource Center and a Complex Care Management team the Lead Care Manager (LCM) will work directing support to Gardner Health Services (GHS) highest risk patients who are experiencing one or more chronic disease conditions, in addition to ongoing medical and social barriers. The Lead Care Manager will be part of an interdisciplinary team comprised of primary care providers (PCP), RN Enhanced Care Manager, Social Workers, Health Educator, and LCMs. In collaboration with this team, the LCM will outreach and provide coordination to a variety of support systems to patients including care coordination and care management services to health plans referrals and GHS internal referrals.

Compensation and level of position placement will be determined based on relevant experience and education.
