Responsibilities :

Plan, develop, organize, implement and direct the Social Service programs of the nursing home.

Supervise and direct the employees of the Social Service Department.

Assure that Care Plans and documentation are updated and accurate.

Act as the liaison between the Interdepartmental Team and the Social Workers to communicate important information between them

Requirement :
