Requirements :

Valid Licensed Practical Nurse (LPN) license in the State of Ohio

• Long Term Care Experience preffered but not required.

Duties :

Provide direct patient care, including administering medications and treatments as prescribed.

Monitor and document patient health and progress.

Collaborate with the healthcare team to develop and implement individualized care plans.

Assist with daily activities, ensuring patient comfort and safety.

Maintain accurate and timely documentation in compliance with facility and state policies.

Communicate effectively with residents, families, and team members.

Adhere to infection control protocols and safety guidelines.
