Position Title : Social Worker – LCSW

Location : Hyannis, MA 02601

Contract Duration : 13 Weeks

Schedule : 32 hours per week | 3 : 00 PM – 11 : 00 PM | Rotating days, every other weekend & holiday (Emergency Department)

Pay Rate : $50 – $56 per hour

Position Overview :

"Navitas Healthcare, LLC" is seeking Social Worker – LCSW  for an exciting job in Hyannis, MA.

Responsibilities :

Conduct comprehensive clinical assessments for individuals presenting with acute care needs in compliance with Department of Mental Health (DMH) standards.

Develop and communicate initial treatment plans to the multidisciplinary team.

Lead treatment and discharge planning in collaboration with psychiatrists and other clinical staff.

Provide group therapy, individual, and family counseling sessions.

Support patient care planning to promote recovery and continuity of care post-discharge.

Coordinate community services and resources to meet ongoing medical and psychosocial needs.

Assist patients and families with transitions to home care or appropriate facilities.

Act as a liaison between patients and community agencies to ensure access to appropriate services.

Deliver crisis prevention and intervention support as needed.

Communicate with managed care organizations to secure necessary authorizations.

Maintain compliance with all CMS, DPH, and DMH regulations.

Participate in community outreach and educational activities.

Provide coverage for inpatient and partial hospitalization behavioral health programs as needed.

Qualifications :

Active Massachusetts LCSW license required

BLS Certification (Basic Life Support)

Crisis Prevention Intervention (CPI) training

Strong communication and documentation skills

Experience in acute behavioral health, crisis intervention, or emergency department settings preferred
