Must have active Maine license. Must have Home Health experience! Must have 2+ references in profile to be submitted. 90-mile radius rule.

Required Qualifications

Physical Therapist, Home Health

Experience : 1 year

Licenses : PT-ME

Certifications : BLS-AHA(Copy needed)

SSN Required

DOB Required

Work History : Should not be more than 90 days of gap in last 1 year

References : 1 Reference in entire work history

Graduation from an accredited physical therapy program

Valid driver's license to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy
