Hospice Social Worker

Overview : We are seeking a dedicated Hospice Social Worker to join our compassionate team.

The Hospice Social Worker plays a vital role in our interdisciplinary team, providing essential psychosocial support to patients and their families. This position involves assessing the unique needs and goals of each patient / family unit, implementing interventions, and ensuring that their care aligns with established plans.

About You :

• Bachelor's degree in Social Work from an accredited institution.
• Licensed as required by state regulations.
• A minimum of one year of experience as a Social Worker in healthcare or hospice settings; three to five years is preferred.
• In-depth knowledge of terminal illness and its impact on patients and families, with a keen sensitivity to life and death issues.
• The ability to support families during times of crisis and manage the stress associated with loss.
