Conduct skilled nursing home visits and patient assessments
Lead Start of Care, ROC, and discharge visits (OASIS)
Deliver wound care, medication management, IV therapy, teaching
Manage your patient panel with RN case ownership
Coordinate care with LPNs, PT/OT, and office team
Chart visits in WellSky (Kinnser) within required timelines
Escalate patient condition changes quickly
Active Virginia RN license (required)
3+ years nursing experience
Knowledge of Medicare home health regulations
OASIS documentation experience
Valid driver's license and insured vehicle
CPR certification
Prior home health RN experience
Experience with WellSky (Kinnser) EMR
Strong case management skills
Pay: $85,000–$91,000/year + productivity bonus potential

Local travel: Newport News, Hampton, surrounding areas
