Please note: All opportunities at 21HHS require being in the field visiting patients in their homes. Remote/work-from-home positions are not available unless specifically noted.

Territory: Solano County

Under the direct supervision of an RN Case Manager, you will provide patient care according to the Plan of Care.

What You'll Do as a Certified Home Health Aide (HHA):

• Provide personal care including, baths, back rubs, oral hygiene, shampoos, and changing bed linen
• Assist patients with dressing and undressing
• Provide skin care to prevent breakdown
• Assist the patient with toileting activities
• Keep patient's living area clean and orderly, as appropriate
• Assist in feeding the patient, if necessary.
• Perform range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
• Assist patient in the self-administration of medication.
• Meet safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
• Report on patient's condition and significant changes to the assigned nurse.

To Be a Success as a Certified Home Health Aide:

• Valid CHHA CA Certification
• Current BLS/CPR Certification
• Valid CA Driver's License
• Excel at building rapport with patients and providing the highest quality of care
• Experience home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired

• Prior to scheduling an in-person interview, all Candidates are required to complete a computer literacy assessment. This is to ensure you are set up for success and can navigate in our environment including electronic charting and use of our web based communication application. The results of the assessment are confidential and will be used as one of multiple data points that go into the hiring decision process.

$55,000 - $90,000 a year

Estimated Wage range Certified Home Health Aide (CHHA): 55,000.00 - 90,000.00 Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match).

You will be assigned a primary location; however, you may occasionally be asked to provide coverage outside of that area. This may include supporting vacancies, leaves of absence, higher patient census, or other client care needs. While 21st Century Home Health makes every effort to maintain your primary territory, temporary reassignments may be necessary to ensure safe, high-quality patient care and to meet the needs of our growing organization.
