Job Details We are seeking a compassionate and motivated Occupational Therapist (OT) to provide temporary coverage for a maternity leave at a Union City-based care center. This role focuses on delivering high-quality, patient-centered care to a geriatric and frail population, both on-site and through occasional home visits. This is a hands-on, collaborative role within a multidisciplinary care team, offering strong exposure to holistic patient care (rehab, mental health, dietary, etc.). Key Responsibilities: • Provide occupational therapy services to elderly/frail patients • Develop and implement individualized, care plan-driven treatment plans • Conduct home safety evaluations and patient home visits (approx. 4–5/month) • Collaborate closely with: ○ Other Occupational Therapists ○ Physical Therapists ○ Cross-functional care teams • Maintain accurate documentation (Epic EMR preferred) • Support continuity of care during maternity leave coverage Must have: Active Occupational Therapist (OT) license (California) Graduate of an accredited Occupational Therapy program Reliable transportation and a driver's license (for 4-5 local home visits/month) Preferred: Bilingual in English + Mandarin or Cantonese Experience working with geriatric or frail populations (1–2+ years preferred) Interested in hearing more?
