We are seeking a compassionate and highly skilled RN Case Manager to join our dynamic team of care coordinators within an acute care setting. This role is pivotal in ensuring seamless transitions of care and optimizing patient outcomes across a variety of specialized units, including the Emergency Department (ED), Intensive Care Unit (ICU), Inpatient Treatment Areas (ITA), Pediatrics, and other Specialty Units. As part of a collaborative unit of Case Managers, the RN will focus on care coordination and discharge planning, guiding patients and families through the continuum of care and facilitating movement to the next appropriate level of care.
ResponsibilitiesThis role is based in a Level II Trauma Center with 571 total staffed beds across both campuses. The position follows a 5x8 schedule from 7:30 AM to 4:00 PM. Attire is business casual, and parking costs are covered by the facility.

Pay and BenefitsThe pay range for this position is $45.00 - $55.00/hr.
