The Assertive Community Treatment Team (ACT) provides a high level of outpatient services to clients experiencing Severe and Persistent Mental Illnesses (SPMI) and who have many case management needs. Working primarily in the community, the goal of ACT level of care is to provide “wrap around” support to clients to help avoid inpatient level of care, avoid criminal justice involvement or police contacts, and / or to assist with transition from hospital level of care to the community. ACT clinicians serve as part of an interdisciplinary team that works together to meet the needs of clients within the framework of the CO-ACT Model of care.

Education, Knowledge, Skills & Experience Required :

• Conferred Master’s Degree in social work, counseling, psychology, or related field required.
• Registration with DORA required (LSW, SWC, MFTC or LPCC).
• LPC, LP, LCSW, LMFT preferred.
• CAS or LAC preferred (Licensure supervision provided for LCSW, LPC, and LAC candidates)
• Motor Vehicle Insurance Coverage requires applicant must be 25 years or older. Must have an acceptable Motor Vehicle Driving record as determine by our insurance carrier. An MVR will be pulled along with a background check.
• Bilingual (English / Spanish) preferred

Salary Grade 51-Master’s Unlicensed Clinician $57,000 - $64,000

Salary Grade 52- (Licensed Clinician LCSW, LMFT, LPC) $70,000 – $77,200
