Responsibilities:

• Conduct comprehensive assessments of residents' physical, mental, and emotional health to develop and implement individualized care plans, ensuring optimal patient outcomes.
• Provide prescribed medications and treatments, monitor for side effects or reactions, and ensure compliance with treatment plans for chronic conditions, rehabilitation, or post-acute care.
• Continuously monitor residents' vital signs, wounds, and overall conditions, providing appropriate interventions and support, and reporting any changes to the healthcare team.
• Work with physicians, therapists, and other healthcare professionals to coordinate care and provide education and emotional support to families on patient progress and care requirements.

Qualifications:

• SC Registered Nurse license
• Basic Life Support (BLS) certification
• 1-2 years of experience working in a skilled nursing facility or with geriatric populations
