Transitions Healthcare
The Marketing & Business Development Liaison is an independent, autonomous role responsible for driving patient census growth through referral development and relationship management. This position serves as the primary external representative of Transitions Healthcare, building and maintaining strategic partnerships across the healthcare continuum.
The Marketing & Business Development Liaison is the face of Transitions Healthcare in the market. This individual is responsible for developing and managing a strong book of business by cultivating relationships with hospitals, inpatient rehab facilities, skilled nursing facilities, assisted and independent living communities, and home health agencies. Success in this role is measured by admissions growth and sustained referral partnerships.
Develop and grow patient census through targeted outreach and relationship-building with:
Hospitals
Inpatient rehabilitation facilities (IRF)
Skilled nursing facilities (SNF)
Assisted and independent living facilities (ALF/ILF)
Home health agencies
Serve as the initial point of contact for Transitions Healthcare referral partners
Coordinate referrals with Intake and internal teams to ensure the admission process is initiated efficiently
Plan, develop, and execute regional marketing and public relations initiatives
Assist the Director of Sales & Marketing with public relations strategies and initiatives
Establish and maintain strong collaboration between office staff, marketing team, and medical providers
Ensure all public-facing materials accurately represent the practice, including:
Policies and procedures
Marketing collateral
Brochures and information packets
Support and promote the mission, values, and objectives of Transitions Healthcare
Represent the company at meetings, networking events, in-services, and community events
Schedule and participate in developmental meetings related to marketing and public relations
Maintain an organized and current book of business, including referral contacts and activity tracking
Participate in weekly team meetings and monthly performance reviews
Associate’s or Bachelor’s degree from an accredited college or university and:
Minimum of 5 years of experience in marketing and business development
OR 5 years of experience in healthcare marketing/business development within:
Home health
Mobile medical practices
DME
Hospitals or related healthcare fields
Valid driver’s license and reliable transportation
Proven experience in healthcare sales and relationship-based marketing
Strong understanding of insurance models, including Medicare and commercial insurance
Ability to work independently, generate leads, and close referral relationships
Strong communication, organizational, and time-management skills
