RN Complex Case Manager

A company is looking for a Complex Case Manager III to provide member-centric care and coordinate services across the healthcare continuum.

Key Responsibilities

Conduct comprehensive assessments and develop individualized care plans for members

Collaborate with members, families, caregivers, providers, and interdisciplinary teams to deliver holistic care

Monitor and coordinate services to reduce illness burden and healthcare costs for individuals and families

Required Qualifications

Bachelor's degree in Nursing or equivalent work experience, plus 5 years of clinical / acute care nursing experience

Current, unrestricted RN license in the state of residence

Experience working with vulnerable and complex populations in clinical, home care, or telephonic environments

Knowledge of managed care principles and regulatory guidelines

Proficiency in electronic health records and care management software
