Pennsylvania Licensed Practical Nurse

A company is looking for a Remote Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for a day shift position.

Key Responsibilities

Manage patient census and close gaps in clinical and non-clinical patient care

Identify and coordinate community resources for patients to enhance their care

Provide patient education on chronic condition management and perform medication management

Required Qualifications

Active Multi-State / Compact License (LPN) or (LVN)

At least 3 years of direct patient-facing work experience

Technical aptitude with Microsoft Office Suite

Ability to work in a sedentary position for extended periods

Must have a home office or HIPAA-compliant workspace
