Position Details

Merakey Community Case Management is seeking an LMHP to conduct clinical intake assessments and re assessments as needed for the nursing home diversion and transition. Assessments may be completed via telehealth but may also be completed in person as needed. Knowledge of eligibility criteria for community mental health services is preferred, specifically Intensive In-Home, Mental Health Skill-Building, and Community Stabilization.

Requirements

The ideal candidate will possess the following qualifications:

• Master’s degree in human services
• LPC/LCSW required
• Valid driver's license required
