The Residential Nurse Case Manager plays a vital role in ensuring the health and well-being of individuals with intellectual disabilities in residential group homes and Adult Services Programs. This position involves developing and managing individualized healthcare plans, coordinating medical care, and collaborating with interdisciplinary teams to support holistic health outcomes.

What You'll Have :
Licensed Practical Nurse (LPN) certification in PennsylvaniaAt least 1 year of clinical experience in acute or long-term carePreferred experience working with individuals with intellectual disabilitiesCPR and First Aid certification (preferred)Ability to manage confidential information with discretionStrong organizational and time management skillsA Typical Day May Include :
Visiting group homes to assess health and safety needsCoordinating medical appointments and transcribing physician ordersMonitoring medication administration and liaising with pharmaciesConducting quarterly nursing assessments and staff trainingsParticipating in interdisciplinary meetings and care planningMaintaining comprehensive medical records and ensuring timely communicationResponding to after-hours calls related to electronic medication records (eMAR)Attending in-service programs and completing continuing education requirementsPerks with a Purpose
