You will develop referral relationships; you’ll play a fundamental role in helping us grow through patient referrals. To help support our business goals, your primary aim will be to foster strong referral relationships within a geographic territory. You will develop and maintain excellent relationships with all stakeholders including prospective patients, family members, physicians, Hospitals, ICU and floor nurses, discharge planners, case managers and payer representatives. Bright Heart Hospice,

• Develop strong relationships within an assigned territory, prioritizing face-to-face connections.
• Assist in improving the referral-to-admission process.
• Educate community, referral sources and physicians on our hospital programs and services.
• Utilize market analysis to identify new opportunities.
• Responds to and overcomes admission barriers and follows-up on admissions variables.
• Conduct in-services and professional presentations to various groups about our services and outcomes.
