What You'll Do

Own your patient panel across partnered SNFs

Manage chronic conditions and complete full assessments

Close care gaps (HEDIS quality measures)

Lead care planning with your IDT

Keep patients and families informed

Document fast with modern tools

Reduce avoidable hospital use

Must-Haves

Active, unrestricted APN license in South Carolina

Board certified + Medicare credentialed

6+ months SNF or long-term care experience

Comfortable with digital tools

Able to travel daily to multiple SC facilities
