Job Summary
The Patient Navigator is accountable for facilitating patient continuity of care with the healthcare team. Acts in an intake model, assessing the patient needs, assisting with planning, organizing, implementing and evaluating the patient care needs with the healthcare team. Functions in direct support of the Hospitalist team and in partnership with other members of the healthcare team as a patient advocate, educator and coordinator.
Minimum Job Requirements:
Three years of experience as a Registered Nurse required. Previous case management experience or related experience preferred.
- License/Registration/Certification
Current TX/Compact RN License required.
Current ACLS and PALS required.
Bachelor’s Degree in Nursing required.
- Skills:
- Excellent leadership, recommendation making and delegation skills.
- Knowledge of care management, resource and utilization management.
- Skilled in care management and patient assessments.
- Excellent verbal and written communication skills required. Must be able to interact comfortably with physicians, nurses, clinical staff, patients, families and community agency representatives.
- Ability to monitor, assess and record patient progress against a plan of care.
- Ability to facilitate patient access to community resources.
- Ability to work collaboratively with both internal and external medical staff and external partners and organizations.
- Ability to assess, adjust, and calmly respond to changing and crisis environment.
- Ability to maintain confidentiality with all aspects of patient information in accordance with all applicable policies and regulations.
- Bilingual English/Spanish Preferred
