Licensed Clinical Social Worker

A company is looking for a Licensed Clinical Social Worker (Population Health).

Key Responsibilities :

Conduct thorough biopsychosocial assessments to determine patient needs and appropriate interventions

Provide evidence-based individual, group, and family therapy, and develop personalized care plans

Coordinate care and services for patients, collaborating with multidisciplinary teams and maintaining documentation

Required Qualifications :

Master's Degree in Social Work from an accredited school

2 years of clinical experience in care management or behavioral health

Experience in social work counseling with complex psychiatric or substance abuse disorders preferred

Licensed Clinical Social Worker upon hire

Less than 1 year of experience in health coaching and motivational interviewing techniques preferred
