Community Health Physician

We are seeking collaborating physician (MD/DO) contractors to support our clinicians performing In-Home Health Evaluations (IHEs). As a physician collaborator, you will earn a monthly stipend based on the number of nurse practitioners and/or physician assistants you supervise.

• Must be licensed to practice medicine in both North Carolina and South Carolina.
• Out-of-state clinicians need to complete at least one in-person evaluation day or conduct a minimum of 5 in-home visits annually in South Carolina to qualify as a South Carolina collaborator.
• Travel support for airfare, hotel, and rental car may be available, contingent on eligibility.

Interested in conducting in-home evaluations?

Our clinicians provide personalized IHEs to health plan members, including a detailed medication and medical history review, as well as a physical assessment conducted at the member's home. You will receive a patient-specific form for each evaluation, and please note that prescribing medications, ordering lab tests, or altering treatment regimens is not part of this role.
