[job_card.job_description]Assists HCH Director to develop and enhance wellness initiatives, health education groups, activities for consumers, staff and community. Monitor critical chronic disease indicators by utilizing Population Health reports in Care Manager for assigned caseload. Identify high risk consumers through annual health screening and assessment and make referrals if needed. Completion and tracking of all required assessments and screenings for each consumer on caseload. Maintain an accurate and up to date consumer record as outlined in the agency and site Policies and Procedure Manual. Adherence to all applicable evidence-based practice models. Orally communicate information effectively and accurately. Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care. Abide by program and agency policies and procedures Other job duties and special projects as assigned.

Job Purpose
The Healthcare Home Nurse Care Manager will assist the CMHC and / or CPRC with the implementation and integration in daily clinical operations of Healthcare Home and its computer-based disease management programs.
Qualifications
- Possession of a High School Diploma or equivalent;
- Registered Nurse or Licensed Practical Nurse with current Missouri license.
- Five years primary care experience in Nursing and experience with healthcare technology preferred.
- Meet the minimum requirements to be clinically privileged by FCC Behavioral Health.
