A company is looking for a Client & Caregiver Coordinator.
Key Responsibilities
Act as the first point of contact for clients and families, managing inquiries and scheduling needs
Conduct client onboarding assessments and develop initial care plans
Support caregivers through coaching and ensure client satisfaction through regular check-ins
Required Qualifications
At least 2 years of experience in a licensed home care organization
At least 2 years in a care coordination role or similar
Bachelor's degree preferred, but not required
Ability to work 100% remotely and manage a high-volume workload
Experience with documentation and record-keeping for continuity of care
